The center closed down and the town’s people try to make the doctors feel better by giving them busy work. Jeong-Seon calls Ju-Cheon because she’s in a lot of pain; he suggests they get an emergency boat but she refuses out of fear of being found out. They tough it out until the 1st ferry departs in the morning.
The next morning, when the doctors wake up, they head outside and discover Byeong-Cheol drunk and going on his bike when he gets hit. He is thrown to the curb and suffers bad injuries to his face. The doctors end up breaking the law to treat him.
Jeong Seon ends up suffering from an ectopic pregnancy and loses her child; she says she’s pathetic because she thinks its a relief this happened. She says harsh words towards him, but he asks her not to be so harsh because he’s hurt too. He also admits he was scared because it all happened so fast.
The doctors on the island end up getting questioned by the police because they broke the law treating a patient, and Ms. Hwang takes the fall because she thinks she should be the one who does it.
Hyang-Mi ends up coming to their aid and suggests to the doctors to bow down the governor and just aid him in his re-election campaign and they’d be able to get back the center. Ji-Ui refuses to accommodate despite everyone around him being in agreement.
Byeong-Cheol goes back to the center and is seen hanging around by Ji-Ui, who checks up on him and asks him if he’s doing better. Byeong-Cheol asks Ji-Ui why he helped him despite him being a pain to deal with and partly causing the center shut down; Ji-Ui says its important to treat patients fairly in spite of everything. Byeong-Cheol ends up handing him evidence of the governor’s corruption, and Ji-Ui exposes him live as he campaigns. He is furious and makes a big fuss until he slides as he tries to escape the reporters and hurts his knees badly. He then complains of the pain and Ji-Ui refuses to treat him because the center is closed; he says to open it and receive treatment.
Time goes by, and the center operates as normal; Ms. Hwang’s eye surgery is near, and everyone hears of the governor’s arrest and indictment. They even get funds to replace old machines and celebrate this. However, they soon remember all of them will have to leave as the year is up.
Because of this, they celebrate with the island’s people.
Soon, Chi-Yeon prepares to leave but takes all of the gifts by the island people saying he’s changed. He still does not know what he’ll choose as a doctor but has a lot of work. He thanks Ha-Ri and they shake hands, parting ways.
Jeong-Seon discusses with Ju-Cheon, preparing for their inevitable breakup because he’s leaving but reveals she’ll also try to follow him to where he is, they end up kissing and spending the night together.
Ha-Ri is told by Ms. Hwang that Ji-Ui has been raising hell trying to stay on the island but he can’t. She tells her she should also follow her dreams and not be stuck here. Then, she ends up watching an old video of her grandma talking about her strong granddaughter; she cries watching her talk about her while taking her last photo.
Ji-Ui and Hari end up going to a different island to eat food and on the ride there, she tells him she’s going to go to the hospital and following along, he’s very happy to hear this and the couple end up kissing.
Then Ji-Ui says in voiceover that he’s been assigned to another strange place, this time, he’s assigned to a prison.
The last scene shows the center’s doctors and nurses taking a group photo together.
