“Resident Playbook” Ending Explained

The team of 1st year residents all meet up for beer and Yi-Young reveals she’s dating Ku Do Won but asks them to keep it a secret. However, soon at the hospital, several different doctors and nurses are talking about it, apparently, everyone knows.
Ku Do Won is at the bank about his accounts and says he wants to open a one-year savings account thinking he’ll need it soon for wedding preparations. The 1st year residents catch him and make fun of him calling him brother in law.
Oh Joo-Young gets invited to her friend’s 1st kid’s birthday and makes an excuse to not go. She’s very upset and hopes she’s pregnant soon.
Oh Yi-Young checks the lotto numbers and finds another one hidden in her drawer and check it, discovering she won around 30K won [around 20K USD] and is happy. Yi-Young also tries to remove the placenta from a patient and fails and gets upset over it.
Several women are having babies as the year draws to a close and many wanted a 1st of January baby but didn’t get it. Yi-Young convinces several of them not to delay it and guides them through it. Pyo Nam-Kyung sees an old patient of hers and finds out she’s dying and has an NDR order on her. The patient dies as January 1st comes. She’s very upset over it, cries and then has to snap back to reality as a woman who is delivering comes and delivers her baby. Dr. Yang Seok-Hyeong makes an appearance and is impressed with.
While checking up on a patient refusing to sleep, Um Jae-Il tries several other things to do and makes the patient’s pain drop. Professor Jo praises him and wants to work on a paper with him.
As january 1st comes to an end, Yi-Young brags about winning the lottery but the withdrawal time has passed and she’s lost it. She wanted to quit and not do year 2.
As everybody leaves work at night, Jae-Il and Sa-Bi are walking alone when she asks if he likes her while he talks about something and accidentally slips that she’s cute. He admits to it and she says she needs time to get to know him before dating. He pesters her jokingly because he likes her so much.
Yi-Young meets with Do-Won who gives her flowers for completing her 1st year and then also presents her a couples ring. They share a sweet kiss while it snows.
A time later
Professor Im Dong Ju retires and everyone including Dr. Yang Seok-Hyeong attend the ceremony.
Sadly, Oh Joo-Young finds out she’s not pregnant and decides to just give up on the baby, she will not try anymore.
We find out that Dr. Chu Min-Ha [cameo by Ahn Eun Jim] became the new professor and she’s greeted by everyone. Yang Seok-Hyeong brings flowers and greets her and is embarrassed because others were there too.
The last scene of this drama shows Yi-Young being escorted by her boyfriend Do Won to the hospital again. She is now starting her 2nd year in a different room, she takes a call and happily calls herself ‘2nd year resident Yi-Young.’
